• Works closely with the ALS neurologist
  
  
• Coordinates the efforts of the multidisciplinary team in clinic
  
  
• Coordinates comprehensive home-care services
  
  
• Provides comprehensive patient and caregiver education
  
  
• Serves as patient advocate
  
  
• Refers patients and caregivers to community health care providers, national ALS resources and support groups.
